Section three reads very much like it has been written for an expert audience. This may well be the intention, but it should be stated somewhere. Even for an audience familiar with the issues, some of the document is difficult to follow. In particular, I do not think that section 3.4 is at all clear. In order to understand parts of it (esp 3.4.1) I had to refer to the original material by Endo (2009) which is not actually referenced at this point. I wondered if the work had been cut down for space saving reasons.
